You can sign in from the home page of your survey's website, which will look like the image below, except with your survey's own logos and welcome message:
Forgotten Password
If you forget your password, you can follow the Forgotten Password? link. Here you can enter your User ID and receive a secure link by email which you can use to reset your password.:
If you don't receive the reset password email, please check your spam or junk email folders.
Forgotten User ID
If you have forgotten your User ID, you can follow the Forgotten User ID link. Here you can enter the email associated with your account, and a User ID reminder email will be sent to that address:
